This section contains personal details which will ensure a smooth transition from Year 11 into Sixth Form, including making arrangements for Sixth Form interviews. Please complete all sections.

This section enables you to state clearly what your course choices are. If you are likely to gain at least 5 GCSEs at Grade 4 or above (including some 9,8,7,6,5 grades and including Maths or English at grade 4 or higher), then you can consider Level 3 courses (A level and equivalent). Students taking Level 3 courses should pick three subjects from the selections below (no more than one from each group).
In addition, all students in Sixth Form follow a programme of community service, enrichment and General R.E.
